1/16/2023 EnchantaVaughn Approved 1. Behavior: When QMHP walked in the home. Client acknowledges her progress and states she has been working on speaking positive affirmations daily. Client is aware that her counselor is there to help her maintain her goals and objectives, but she has to do her part as well. Client verbalizes positive communication toward QMHP and exhibits excitement when sharing all of the things she did today that she was proud of. Client had a good attitude and was supported for her cooperation throughout the session. 2. Behavior: Client transitioned smoothly into psycho educational activity. Client communicated positive I statements, "I plan to be good for the rest of the day" to prove she can be responsible and compliant with in-home instructions. Client has been doing a better job adhering to rules and following directions correctly. The interventions were viewed as successful. No further incidents were observed or reported. Client dismissed from session safely. 1. Intervention: QMHP inquired about clientâs day and the things she was doing to make it a good day. QMHP assisted client with recognizing her own personal strengths and how to apply them daily. QMHP continued to monitor clientâs relationship with mother and compliance to duties and expectations in the home and in the community. QMHP encouraged client to use her strategies and skills with peers, family, and mental health advocates to support her with managing symptoms associated with ADHD. 2. Intervention: QMHP utilized active listening as information was conveyed. QMHP continued to engage with client and provided positive reinforcement as she participated in the session. QMHP provided a list of experiences that is common with her mental illness. QMHP challenged client to continue displaying a positive attitude. QMHP provided praise when the client continued to display positive behaviors throughout the session, remaining on task and is very respectful to her mom and brother. 1. Response: Client has presented herself in a responsible way, utilizing her breaks appropriately without displaying attention-seeking behaviors. Client responds well to praise and support. As the client developed understanding of therapeutic expectations, she reported increased satisfaction and function. Client is making progress with restoring relationships in her life and being the example to her peers and sibling. Client focused on things she is good at and loves to do at home in order to stay motivated. 2. Response: Client maintains good etiquette by sitting in her seat during instructed activity, raising her hands, asking appropriate questions, being respectful toward mental health professional, remaining quiet during quiet time, active during social time, listening to his counselor, modeling appropriate behavior, and earning a reward. Client says please, thank you, and excuse me appropriately during the session making it easier for QMHP to document important information related to her mental disorder.
